After you're certified: your Missouri-licensed physician or nurse practitioner submits the certification to the state, and you apply for your patient card through the online registry within 30 days. Have a copy of your photo ID and a current digital photo ready. Cards are valid for three years. Patients under 18 need a parent or legal guardian as caregiver.
